PCP Screening

Visit a primary care physician (PCP) of your choice to complete your annual physical and earn points. Have your physician complete the form and upload to Health Advocate under ‘Annual Physical.’

Maintain Your Discount

Throughout the year, continue earning points and move up to Level 2 or 3 by completing activities that prioritize your health. From taking a walk, getting annual exams, fitness challenges, and more – there are plenty of points earning opportunities. Look at your goals, plan activities to help you reach them, and you will be on your way to Level 2!

How many points do I need?

After you reach 200 points and achieve Level 1, be sure to reach 500 points by June 15th, 2024 to maintain your wellness discount. Once Level 1 is achieved, employees can redeem their points in the rewards mall.

Earn Rewards

Employees and spouses can redeem rewards in the Health Advocate Online Rewards Mall. Redeem $1 for every 2 points earned for up to 600 points. Redeem your points for:

  • E-gift cards from retailers like Amazon and Target, or digital Visa gift cards
  • Fitness devices from Fitbit and more
  • Outdoor recreation equipment like tents, backpacks, and coolers
  • Donations to your favorite charities
  • Home accessories, personal care items, and more
