M.C. Dean employees can save hundreds of dollars per year on insurance coverage by earning points through Go365! To get started and earn Bronze Status, you’ll need to get a wellness biometric screening, sign in to Go365, and complete the annual health assessment.

  • 2022-2023 Wellness Guide

    This guide will provide additional details on Go365 & many features available to you through this wellness program.

  • Visit your Personal Physician, Earn Points

    Employees are eligible for a 350 point bonus once they meet with their personal physician and complete a form confirming their visit.

Biometric Screenings

  • Free Off-site Screenings with LabCorp

    Free screenings are available to employees with medical insurance through M.C. Dean – by appointment – only at select LabCorp locations.

  • Primary Care Physician (PCP) Screening

    Visit a primary care physician of your choice to complete the Go365 biometric screening form. Upload an electronic copy to your Go365 account under ‘Quick Links’ or fax the completed form to 1-877-250-7814.

Move up to Silver Status

Throughout the year, continue to earn points and move up to Silver Status by completing activities that prioritize your health. From taking a walk, getting annual exams, fitness challenges, and more – there are plenty of points earning opportunities. Take a look at your goals, plan activities to help you reach them, and you will be on your way to Silver Status!

Calculators  75 points each (up to 300/year)
Conversations 100 points
CPR/First Aid Certification 125 points each
Donate blood 50 points (up to 300/year)
Nicotine test 400 points
Nicotine free 400 points

Dental exam 200 points (up to 400/program year)
Vision exam  200 points
Flu shot 200 points
Sports leagues 350 points

*Subject to certain requirements and will appear on your points statement if they apply to you

Reasonable Alternative Standard: If it is unreasonably difficult due to a medical condition for you to achieve the standards for the reward under this program, or if it is medically inadvisable for you to attempt to achieve the criteria for the reward under this program, call the number on the back of your Go365 member ID card, and a Go365 representative will work with you to develop another way to qualify for the reward.
